Abstract: Pelvic-ureteric junction obstruction is caused by congenital abnormality that blocks the flow of urine from renal pelvis to ureter among children. It can be detected by antenatal ultrasonography with the presence of hydronephrosis. Some presented after few months of life with urinary symptoms or abdominal mass. Renal ultrasound followed by dynamic imaging is required to diagnose this condition. Open pyeloplasty is done to correct this abnormality to ensure good flow of urine from the kidney to the bladder. We described a 3-year-old boy presented with abdominal distension at the age of 6-month-old. Diagnosis of pelvic-ureteric junction obstruction had been made and open pyeloplasty was performed. Despite corrective surgery, he presented with recurrent obstructive symptoms with evidence of stricture over previous operation site. Restenosis is a great challenge in managing PUJ obstruction. A second pyeloplasty was performed to relieve the obstruction.
