Significance of intraoperative platelet-rich plasma as a storage solution for hair grafts till implantation: a randomized controlled study

Year: 2025
Type: article
Abstract: Background Platelet-rich plasma (PRP) is a popular pre- and post-hair transplant treatment. Objective To determine the efficacy of using PRP as a storage solution during follicular unit extraction hair transplantation. Patients and methods A total of 60 male patients with androgenetic alopecia grade III-VI were divided into two groups: PRP (n=30) and normal saline 0.9% (n=30). The follicular unit extraction procedure was followed, and the grafts were kept in either PRP or saline. Postoperative follow-up appointments were arranged at 2, 4, 8, and 12 weeks to evaluate hair regrowth percentage, thickness, density, transplant type, problems, and patient satisfaction. The results were assessed using digital photography and trichoscopy. The outcomes were assessed by an independent, blinded dermatologist. Results The PRP group showed significantly higher postoperative hair regeneration compared with saline group ( P <0.001). The study groups showed substantial differences in postoperative hair density ( P <0.001). After 2 weeks of follow-up, the PRP group had denser grafts. After 12 weeks of follow-up, 73.3% of the grafts in the PRP group were coarse, versus 20.0% in the saline group. Multiple-hair grafts were demonstrated in 86.7% of the PRP group versus 26.7% of the saline group after 12 weeks. Conclusion Follicle unit extraction hair transplantation results are improved when intraoperative PRP is used as a preservation medium.
