Abstract: Scores ≥4 triggered automated EHR referrals to cardiology and telehealth follow-ups.The intervention was integrated into existing OB/GYN EHR workflows (Epic Systems) to ensure scalability. Results: In a pilot cohort of 1,200 women, implementation of the risk tool increased postpartum lipid profiling by 32% (40% pre-intervention vs. 72% post-intervention) and cardiology referrals by 28% (22% vs. 50%).Median time to cardiology follow-up decreased from 65 days to 21 days, with 78% of referred patients attending at least one telehealth consultation.Among high-risk women, early statin therapy reduced 5-year projected CVD events by 38%, with 68% achieving LDL-C ,100 mg/dL within 6 months. In safety-net hospitals, guideline-concordant screening completion improved by 25%, with notable equity gains among Black and Hispanic women (20% to 45% screening rates).Discussion: This study demonstrates that a simple, EHRintegrated risk stratification tool can bridge the gap between clinical guidelines and real-world practice.By prioritizing high-risk women for postpartum care, health systems can address systemic inequities in CVD prevention. The use of existing EHR infrastructure minimizes implementation costs, making the model scalable for diverse settings.Challenges include ensuring patient engagement in telehealth follow-ups and standardizing biomarker measurement protocols. Conclusion: An automated, evidence-based risk stratification scale offers a pragmatic solution to reduce lifelong CVD risk in women with preeclampsia or GDM.Health systems should adopt similar EHR-driven interventions to align with AHA/ACOG guidelines and improve equitable access to preventive care.
