Abstract: Purpose: Previous studies regarding the time trend of prostate cancer incidence are limited, therefore, the present study aimed to explore the time trend of prostate cancer incidence from 1996 to 2020 in Taiwan. Materials and methods: This study employed data from the Taiwan Cancer Registry spanning 1996 to 2020, including patients diagnosed with prostate cancer. Joinpoint regression analysis was used to assess annual incidence trends. Furthermore, an age-period-cohort (APC) model was applied to investigate the independent effects of age, calendar period, and birth cohort on incidence rates. Results: The prostate cancer incidence rate continued to increase from 1996 to 2011 (age-standardized rate: approximately 15 to 30 per 100,000), stabilized from 2011 to 2016, and rebounded after 2017. The annual percent change was 6.58% (1996–2011) and 3.12% (2011–2020) in men aged ≤64 years, and 5.00% (1996–2010) and 2.44% (2010–2020) in men aged ≥65 years. A shift toward higher-stage disease was observed after 2018. The APC model achieved the lowest deviance (64.0062) among all tested models, and likelihood ratio testing indicated that age and period effects were significant contributors to prostate cancer incidence trends in Taiwan. Conclusion: The APC analysis indicated that the incidence of prostate cancer is related to age, calendar period, and birth cohort. Taking age, period, and cohort into simultaneous consideration is a more informative approach to understanding temporal trends in prostate cancer incidence.
