Immediate dental implant placement in recently extracted tooth sockets with substitute augmentation material and local antibiotic

Study design: Clinical Trial
Abstract: Background. Immediate dental implant placement into fresh extraction sockets offers advantages such as reduced treatment time and preservation of alveolar bone. However, short-term outcomes depend on adequate stability and infection control. The objective of the research was to evaluate the effectiveness of immediate implant placement using a combination of a synthetic bone substitute (Bioplast-Dent) and a topical antibiotic (Lincomycin). Methods. In this prospective clinical study, 52 healthy non-smoking Iraqi patients (aged 20-70), recruited at the College of Dentistry, Al-Esraa University, required tooth extraction and implant placement. Inclusion criteria included good oral hygiene, absence of systemic diseases, and patient consent for CBCT imaging. Following flapless tooth extraction, sockets were debrided and grafted with Bioplast-Dent granules (200-1000 μm) soaked in Lincomycin liquid solution (500 mg/2 mL). Nucleoss T6 dental implants were placed immediately into the prepared sockets. Primary implant stability was confirmed, and patients were followed clinically at 1 week, 1 month, and 3 months. Osseointegration was assessed radiographically using 2D-OPG or 3D CBCT imaging before final prosthetic loading. Statistical analysis was performed using the Chi-square test, with significance set at p < 0.05. Results. A total of 52 implants were placed. After 3 months of follow-up, 49 implants were successful and 3 failed, resulting in a 94.3% success rate (p = 0.004). Failures were mainly associated with low bone density (D3 type). No major complications or infections were reported during the 3-month follow-up period. Conclusion. Dental implants placed immediately after the introduction of a GBR conjugate antibiotic into selected extraction sockets have high survival rates comparable to delayed implants placed in healed sites. Immediate implants offer significant benefits and a high success rate. While these preliminary results are encouraging, the absence of a control group and the limited 3-month follow-up restrict the strength of the conclusions. Further controlled studies with larger samples and longer observation periods are recommended to validate these findings.
