Abstract: Patient was a known case of bipolar affective disorder since 20 years and was taking tablet lithium carbonate 450 mg twice daily.Patient had also been diagnosed with hypertension 6 months back and was taking tablet telmisartan 40 mg daily along with thyroxine 100 mg tablet for hypothyroidism since 6 months.He became irritable, dyspneic, agitated, and was shifted to the intensive care unit (ICU).His heart rate was 108/ minute, and he maintained a blood pressure of 81/61 with noradrenaline support.His respiratory rate was >35/minute, 1-
