Abstract: Tibiofibular fractures are often complex and involve both bone and soft tissue structures, making accurate diagnosis essential for appropriate surgical planning and better recovery outcomes. This paper explores current diagnostic techniques of complex tibiofibular fractures, particularly focusing on the application and advancement of CT and MRI imaging. The results of this paper show that conventional X-rays have significant shortcomings in terms of diagnostic sensitivity and accuracy. Multidetector computed tomography (MDCT) with three-dimensional (3D) reconstruction significantly improves fracture classification and spatial visualization, whereas magnetic resonance imaging (MRI) enhances soft-tissue and ligament assessment, further illustrating the importance of multimodal diagnostic planning for complex tibiofibular fractures. In addition, artificial intelligence-based image interpretation and CT-MRI fusion techniques show potential for improving diagnostic accuracy, speeding up processing, and evaluating outcomes. By evaluating the advantages and limitations of current techniques, this paper aims to provide a foundation for future developments in the precise and efficient diagnosis of complex tibiofibular fractures.
