Abstract: Acute coronary syndromes (ACS) can occasionally present with subtle or even absent electrocardiographic ndings, leading to diagnostic delays. We report a case of proximal left anterior descending artery (LAD) occlusion in a 57-year-old male who presented with atypical ndings, including a normal initial ECG and cardiac markers. Early bedside echocardiography and high clinical suspicion played a critical role in the diagnosis and management.
