Abstract: A bstract Introduction: Split cord malformations (SCMs) are developmental anomalies that are associated with a number of congenital defects. However, a combination of SCM I with a dermoid cyst is very rare, and very few cases have been described in the literature. To the best of our knowledge, such a type of co-existence of multiple congenital spinal anomalies with delayed presentation has been reported by very few literature studies. Case Presentation: We report a case of the above combination in a 11-year-old female child having SCM I with dermoid cyst, butterfly vertebra, syringomyelia, thickened filum terminale, sacral spina bifida, and tethered cord. Conclusion: Complex SCM may rarely present with multiple congenital spinal anomalies. Careful evaluation of neurological findings along with radiological features is important in formulating the proper surgical strategy. Surgery for lesions causing symptoms or that are likely to be symptomatic later in life should be done as early as possible.
