Restoring Guidelines: EBM-X to Bring Critical Thinking and Humility Back to Evidence-Based Medicine
Year: 2025
Type: editorial
Abstract: Evidence-Based Medicine (EBM) revolutionized clinical care by emphasizing the integration of best available evidence with clinical expertise and patient values. Yet, over time, EBM’s critical foundations have eroded into rigid protocols, diminishing context and clinical reasoning. This editorial examines how uncritical adherence to evidence—especially during crises like COVID-19—can lead to iatrogenic harm, overdiagnosis, and misplaced certainty. It highlights structural issues such as the imbalance between efficacy and safety in trial design, the hierarchy fallacy, and the neglect of cognitive biases in clinical decision-making. To restore EBM’s transformative potential, we propose EBM-X: a reflective model grounded in complexity, epistemic humility, and patient-centered reasoning. EBM-X reframes evidence as a tool—not a command—supporting medicine that is as thoughtful as it is scientific.
