Evaluation of Pulse Oximeter Derived Perfusion Index as an Objective Tool for the Assessment of Analgesia during Head and Neck Oncosurgery under Anesthesia

Abstract: Introduction: Assessment of analgesia under anesthesia is challenging because precise assessment of the adequacy of analgesia is difficult.A nociceptive surgical stimulus causes sympathetic nervous system activation, causes vasoconstriction, and a decrease in perfusion index (PI).The aim of the study was to assess the PI derived from a pulse oximeter for the assessment of pain to surgical stimulus under anesthesia in head and neck oncosurgery.Materials and methods: A total of 22 patients scheduled for head and neck oncosurgery were included in the study.General anesthesia was administered.Vital parameters like PI, heart rate (HR), and mean arterial pressure (MAP) were monitored and recorded at frequent time intervals-before induction, during induction, after intubation, and after skin incision; at 2 minutes, then every 5 minutes up to 30 minutes.Data were analyzed using paired t-tests.Results: PI values after fentanyl increased from 1.24 ± 0.29 to 4.45 ± 3.37 with p < 0.001, HR decreased from 98.35 ± 17.05 to 89.35 ± 15.56 (p < 0.05), and MAP decreased from 108.36 ± 17.45 to 95.63 ± 14.96 (p < 0.001).The increase in PI values after skin incision correlated with the decrease in HR and MAP up to 30 minutes. Conclusion: PI could be an indicator of nociception assessment to surgical stress and response to analgesics under anesthesia.
