Abstract: Phosphodiesterase type 5 inhibitors (PDE5Is), used primarily to treat erectile dysfunction and pulmonary hypertension, are gaining attention for their potential off-label applications and new prospects for their use in various fields of medicine. Their current off-label indications are widely used in cases such as cardiovascular and pulmonary diseases, and Alzheimer's disease. In oncology, PDE5Is have shown promising effects in increasing the efficacy of traditional chemotherapy. By modulating immune responses and increasing drug sensitivity, these drugs may be helpful in treating various cancers. In obstetrics and gynecology, PDE5Is are being studied for their ability to improve placental blood flow, which may have potential in treating restricted fetal growth, and in improving the ability of the endometrium to accept an embryo, which could aid in the treatment of infertility. In diabetes, PDE5Is have been shown to improve endothelial function, glucose metabolism, and vascular health, with potential benefits in treating complications such as diabetic neuropathy and erectile dysfunction. These promising applications underscore the growing interest in PDE5Is beyond their traditional indications. However, clinical evidence is still limited, with most data coming from preclinical studies and small trials. Further controlled studies are needed to better understand the safety, efficacy, and long-term outcomes of these therapies in the context of indications other than erectile dysfunction.
