Abstract: Sinonasal hemangiopericytoma (SH) is a vascular tumor that accounts for less than 0.5% of all sinonasal tumors. Due to the nonspecific nature of its imaging characteristics, its diagnosis relies on anatomopathological and immunohistochemical evaluation. The gold-standard treatment is complete tumor resection with disease-free margins. Despite its indolent nature, it has a tendency to reoccur locally, requiring a long-term follow-up. A 52-year-old male patient presented with a one-year history of progressive nasal obstruction and recurrent unilateral epistaxis, along with a lobulated mass visible on anterior rhinoscopy. A computed tomography and magnetic resonance imaging of the paranasal sinuses were performed, but the diagnosis remained inconclusive. Complete endoscopic resection of the lesion was carried out, and the surgical specimen was sent for anatomopathological and immunohistochemical analysis, which confirmed the diagnosis of SH. Despite multiple published case reports, challenges in the early diagnosis of SH and controversies regarding its optimal therapeutic approach persist.
