Abstract: Background: Chronic obstructive pulmonary disease (COPD) acute exacerbation is a period of worsening of a previous condition that affects the patient's clinical outcome. Its prevalence in the Asia-Pacific region reaches 46%. The purpose of this study was to determine the relationship between smoking history, exacerbation history, hospital admission history and baseline SaO2 factors with mortality due to acute exacerbation of COPD at Ngoerah Hospital. Methods: This study is an analytical case control study with a sample size of 48 cases and controls each groups. The data collection technique uses the total sampling method. Results: The study sample was dominated by men (62.5%), normal body mass index (BMI) (74%), low baseline SaO2 characteristics <92% (63.5%), history of hospitalization (86.5%), smoking history (55.2%), history of exacerbation (43.8%), and suffering from respiratory tract infection (77.1%). There was a significant relationship between baseline SaO2 and death (OR 5.12; 95% CI 2.03 - 12.83; p <0.05), history of exacerbation and death (OR 4.11; 95% CI 1.73 - 9.71; p <0.05). There was a non-significant relationship between history of hospital admission and death (OR 1.72; CI 95% 0.51 – 5.69; p >0.05), history of smoking and death (OR 1.08; CI 95% 0.48 – 2.43; p >0.05). Conclusion: There is a relationship between baseline SaO2 and history of exacerbation with death. However, there is no significant relationship between smoking history, history of hospitalization, and baseline SaO2 with death due to acute exacerbation of COPD at Ngoerah Hospital.
