Abstract: Thoracic endovascular aortic repair has gained widespread acceptance due to its high success rate and is increasingly being used to treat thoracoabdominal aortic aneurysms (TAAAs). We present a 70-year-old male smoker with chronic obstructive pulmonary disease, hypertension, hyperlipidemia, and poor left ventricular systolic dysfunction diagnosed with a symptomatic Crawford type I TAAA with aneurysm diameter of >5.5 cm and length of 30 cm treated with a total fenestrated endovascular approach with two overlapping thoracic aortic devices after coiling of the celiac axis with fenestration to the superior mesenteric artery. A percutaneous approach from the left subclavian and right femoral artery was used for the procedure. Completion angiogram showed complete exclusion of the aneurysm, indicating good technical success with no evidence of endoleaks. The patient experienced no immediate postoperative complications, had stable vitals, and improved symptomatically.
