Abstract: This case study examines the complex relationship among trauma, substance abuse, mental illnesses, and criminal behavior in the life of Mr. SB, a homeless person. The study utilizes comprehensive narrative analysis to investigate how early childhood trauma, particularly the traumatic loss of his father, initiated a lifelong path characterized by addiction, psychiatric turmoil, social exclusion, and recurrent interactions with the criminal justice system. The story highlights how poverty, systemic inequity, and lack of proper care deepen vulnerabilities, creating a cycle of homelessness, violence, and incarceration. It shows how unaddressed mental illness and substance use impair behavior and social ties. At the same time, intergenerational trauma and family dysfunction perpetuate violence and instability, despite brief periods of clarity and attempts at recovery. SB’s experience highlights the significant challenges associated with dual diagnosis and the limitations of disjointed treatment delivery systems. This instance highlights the essential requirement for coordinated, trauma-informed, and cross-sectoral interventions in psychiatric social work.
