Abstract: True brachial artery aneurysms (BAAs) constitute an extremely rare vascular entity. We present the case of a 68-year-old man with a giant, secondary, symptomatic, saccular, true BAA of the right, dominant arm, measuring 58×55mm. Open surgery was performed under general anesthesia, with aneurysm sac resection and arterial reconstruction with interposition of a reversed great saphenous vein graft. There were no postoperative complications. The patient remains up to date under regular follow-up. The histological analysis was consistent with a true degenerative aneurysm. A comprehensive review of the literature was conducted, highlighting current knowledge on true BAA pathogenesis, clinical features, differential diagnosis, imaging modalities, and treatment strategies.
