Abstract: Takayasu arteritis (TA) is a chronic, granulomatous large-vessel vasculitis predominantly involving the aorta and its primary branches. We discuss the case of a young female who presented with progressive dyspnea, orthopnea, and resistant systemic hypertension. Two-dimensional echocardiography revealed the features of dilated cardiomyopathy (DCMP) with a reduced left ventricular (LV) ejection fraction of 20%–25%. Computed tomography imaging and angiography demonstrated the involvement of abdominal aorta, bilateral renal arteries, and the superior mesenteric artery. Renal angiography revealed severe bilateral renal artery stenosis (RAS). A final diagnosis of TA, complicated by DCMP and bilateral RAS, was established. The patient underwent percutaneous transluminal angioplasty with drug-eluting balloon (DEB) in the left renal artery and bifurcation stenting with kissing balloon inflation in the right renal artery. This case highlights timely intervention in TA leading to reversal of LV remodeling and control of resistant hypertension.
