Background: A recent study has shown that the CXC motif chemokine ligand 9 (CXCL9), a pro-inflammatory protein implicated in age-related inflammation, is associated with poorer musculoskeletal function and increased mortality in older men. We aim to determine the relationship of CXCL9, sarcopenia, muscle strength, physical performance and bone mineral density (BMD) in older women. Methods: A random sample from a prospective cohort of older adults for Biomarker Study for Frailty and Aging (N = 237) was chosen for study subjects that had attended the second clinic visit, and underwent comprehensive geriatric evaluations. Serum levels of CXCL9 were measured by ELISA. Sarcopenia was defined if they had low muscle mass along with either reduced muscle strength or impaired physical performance based on Asian-specific thresholds. Results: Compared to women in the lowest quartile, those in the highest quartile of CXCL9 had slower gait speed (Q1: 1.02 vs Q4: 0.86 m/s); lower grip strength (Q1: 23.51 vs Q4: 19.18 kg) and lower Short Physical Performance Battery (Q1: 11.2 vs Q4: 9.7 points). CXCL9 was negatively correlated with gait speed, grip strength and skeletal muscle index after adjusting for age, body mass index, drinking, physical activity and osteoarthiritis. Women in the highest quartile of CXCL9 had greater likelihood of sarcopenia (odds ratio [OR] Q1 vs Q4: 3.35, 95% confidence interval [CI]: 1.23, 9.11) and weak muscle strength (OR Q1 vs Q4: 6.93, 95% CI: 1.81, 26.57) compared to those in the lowest quartile; p for trend < .05 for all. No significant associations were observed between CXCL9 and spine BMD. Conclusions: Greater serum level of CXCL9 is strongly associated with greater risk of sarcopenia in older women, supporting earlier reports that this chemokine may be an integral biomarker related to both inflammation and aging.
