Abstract: Objectives: This study retrospectively examined changes in body composition following Joripewon-tang-gami administration in obese adults and explored correlations with Ephedrae Herba dosage. Methods: Seventeen adults treated between 2019 and 2025 at the National Medical Center were analyzed using pre- and post-treatment bioelectrical impedance analysis (BIA) data. Paired t-tests evaluated treatment effects, while Pearson correlation and principal component analysis (PCA) assessed associations with cumulative herbal dosages. Results: Treatment led to significant reductions in body weight (-2.01 kg), body fat mass, trunk fat, hip circumference, and BMI, while lean and skeletal muscle mass were preserved. Cumulative Ephedrae Herba dose positively correlated with trunk fat reduction (r=0.488, p=0.0469), though PCA suggested an inconsistent dose-response relationship. Thirteen mild, transient adverse events occurred in six participants, with no serious safety concerns. Conclusion: Joripewon-tang-gami may reduce fat mass without compromising muscle mass and appears safe for obesity treatment. Large-scale, controlled studies are warranted to confirm these preliminary findings.
