Abstract: An 8‐week‐old, entire, male Maine Coon cat was referred with a 2‐week history of urinary tenesmus, pollakiuria, dysuria and intermittent haematuria. Phimosis was diagnosed on physical examination, and a preputioplasty was performed. Complications with the preputioplasty site, suspected to be secondary to trauma, led to a decision to perform a perineal urethrostomy at 9 weeks of age. Recovery from surgery was uneventful, other than two episodes of lower urinary tract infection during the 3 months following the procedure. The cat was neutered at 12 months of age, as neither testicle was normally descended. At the time of castration, the urethrostomy site appeared subjectively proportionate to the patient's size, and a 10‐French Foley catheter was passed through the urethrostomy with ease. At 20 months of age, the cat remains free from any clinical signs, and owner satisfaction is excellent.
