Abstract: This case report presents a complex occlusal rehabilitation of a patient with teeth attrition and retained primary teeth. The patient was a 27-year-old male with a complaint of insufficient space for bonding brackets after visiting the Orthodontics. The patient found mandibular anterior teeth were shorten, several primary teeth were retained and gaps existed between the upper teeth for years. Now, the influence of chewing and appearance, the dental treatment is required. 54, 62, 64, 65, 75 and 85 teeth retained and 12 tooth were loss. 23, 33-43 teeth showed attrition on the incisal end and have limited prosthetic space. X-ray images: 54, 62, 64, 65, 75 and 85 teeth retained and the root of 54, 62, 64 teeth were short; there were no obvious abnormalities in bilateral condyle morphology, periapical and periodontal membranes. By wearing therapeutic occlusal splint, the space for bonding brackets were enough and the vertical distance was properly restored. After the orthodontic treatment was accomplished, 54, 62, 64 teeth were extracted and 12, 14, 22, 24 teeth were restored by implant denture. Finally, partial teeth were restored by veneer. In this case, the combination of therapeutic occlusal splint, orthodontic treatment and implant denture, accurately, minimally invasive and efficient solved the restoration space deficiency, preserved the hard tissue of the tooth to the maximum extent, and met the restoration and aesthetic needs of the patient. Therapeutic occlusal splint is an effective way to address the insufficient space for restorations.
