Abstract: Both in vitro studies and clinical trials have been conducted to assess the retentive force of PEEK and PEKK as materials for secondary crowns in telescopic prostheses. This report outlines a digital workflow for the design and fabrication of a telescopic prosthesis featuring a polyetherketoneketone (PEKK) inner crown and framework. A 56-year-old female patient with multiple missing teeth in the mandible was rehabilitated with a telescopic prosthesis featuring both a PEKK inner crown and a PEKK framework. The treatment process involved intraoral scanning, computer-aided design (CAD), digital milling of the telescopic crown and PEKK framework, and 3D printing of casts. At the 4-year follow-up, the prosthesis demonstrated excellent retention, mastication efficiency, aesthetics, and biological compatibility. Two key clinical outcomes were observed. First, a PEKK telescopic prosthesis can be successfully fabricated using a digital workflow, providing an initial retention value sufficient for intraoral function. Second, over a 4-year follow-up period, no significant wear or reduction in retention force was detected. In this case, the PEKK telescopic prosthesis provided successful restoration of extensive mandibular tooth loss with favorable clinical outcomes. Our findings suggest that PEKK is a viable material for fabricating telescopic prostheses supported by natural teeth.
