Abstract: To investigate the anatomical relationship between periodontitis and maxillary sinusitis in CBCT imaging, and to provide imaging basis for early recognition, multidisciplinary treatment and prognosis evaluation of odontogenic maxillary sinusitis. A total of 60 patients with periodontitis combined with maxillary sinusitis were enrolled in the Stomatology Department of the Sixth Affiliated Hospital of Xinjiang Medical University from March 2024 to March 2025, including 28 females and 32 males. There were 31 cases ≤60 years old and 29 cases > 60 years old. Imaging indicators: the amount of alveolar bone loss. Whether maxillary sinus mucosa thickened or fluid accumulated, sinus floor bone wall integrity. Chi-square test and is used to analyze the correlation between periodontitis and maxillary sinusitis. Maxillary sinus floor bone wall was destroyed in 20% of stage III-IV periodontitis. Maxillary sinus fluid accumulation is more likely to be induced by discontinuous maxillary sinus wall than continuous maxillary sinus wall. Alveolar bone loss > 33% is more likely to induce maxillary sinus effusion than alveolar bone loss ≤33%. Discontinuous sinus wall and stage III-IV periodontitis are the key signs of periodontitis with maxillary sinusitis. Periodontitis in maxillary posterior region can directly induce maxillary sinusitis by destroying maxillary sinus floor bone wall. CBCT can clearly show sinus floor perforation and infection spreading path. CBCT has high diagnostic efficacy in periodontitist-induced maxillary sinusitis, and the amount of sinus floor bone destruction and alveolar bone loss is the core differential index.
