A Case Of Occlusal Reconstruction With Fixed And Implant Restoration

Year: 2025

Abstract: This case involves the treatment of dentition defect and severe full-mouth tooth abrasion. Occlusal reconstruction is carried out through multidisciplinary sequential treatment to address issues such as dentition defect and occlusal dysfunction. The chief complaint of the 68-year-old male is the damage and looseness of the metal-ceramic crowns on the posterior tooth. Intraoral examination reveals severe abrasion of the anterior teeth on the mesial and palatal surfaces. The occlusal surfaces of the posterior teeth crowns are ‌abrade‌d and loose. There is a deep overbite, disorder of the curves. Extraoral examination shows the vertical dimension of lower one-third of face is relatively short, the tension of the masticatory muscles is increased. Diagnosis: dentition defect and severe full-mouth tooth abrasion. Root canal treatment is performed on the ‌abrade‌d anterior teeth. Based on a precise assessment of aesthetics and occlusion, a diagnostic wax-up is designed, and a temporary restoration is completed accordingly. After wearing the temporary restoration for three months, the stable vertical dimension and occlusal relationship are restored by using crowns on the posterior teeth. The reconstruction of the shape and function of the anterior teeth is carried out to achieve a dual improvement. With aesthetics and function, multidisciplinary sequential treatment is carried out for occlusal reconstruction, achieving a systematic improvement from aesthetics to function. By combining the double-barrier post canal technique, a precise base is provided for the restoration of anterior teeth, ensuring marginal adaptation and long-term color stability.
