Abstract: Introduction This case report presents an innovative approach combining 3D-printed individualized titanium mesh (3D-PITM) with autogenous bone ring grafting to reconstruct severe horizontal alveolar bone defects. This technique enhances structural stability and promotes bone regeneration in cases with insufficient residual bone width. Case description A patient with missing teeth at sites #23 and #24 exhibited severe horizontal alveolar bone deficiency, with a residual ridge width of less than 3 mm. A treatment combining 3D-PITM and autogenous bone ring grafting was applied. A ring-shaped autogenous bone block was fixed horizontally onto the alveolar ridge. The 3D-PITM was then utilized to provide structural support and restore the three-dimensional alveolar contour. After 6–9 months of healing, dental implants were successfully placed in the augmented sites. Discussion In cases where the residual alveolar ridge width is less than 3 mm, autogenous bone grafting alone, despite its well-known osteogenic properties, is often insufficient for achieving ideal ridge morphology. While, 3D-PITM effectively restores the three-dimensional structure, the limited bone marrow content and predominant cortical bone in the residual ridge restrict its osteogenic potential. Therefore, combining the biological advantages of autogenous bone ring grafting with the structural support of 3D-PITM maximizes the benefits of both techniques, leading to improved bone augmentation outcomes. Conclusion/clinical significance This case highlights the clinical value of combining 3D-PITM with autogenous bone ring grafting for managing severe horizontal bone deficiencies, providing a predictable bone augmentation approach for complex alveolar defects.
