Abstract: The principle of eliminating infection and restoring function should be followed in the treatment of perimplantitis. When biological complications occur in the one-stage transarch fixed prosthesis, the improper selection may induce complications of other sites. Different methodsshould be adopted with different lesion degrees to ensure the long-term stable function of the whole mouth prosthesis. We report a 14-year follow-up of a patient with terminal dentition after total implant fixation repair. During this period, the patient's 14, 33, and 41 dental implants showed different degrees of periimplantitis, and the periimplantitis recurred after short-term results were obtained through non-surgical treatment. After 13 implant amputation, 33 and 41 implant flap cleaning combined with implant tissue reconstruction and implant surface plasty, the results were satisfactory. The infection was eliminated and the tissue reconstruction around the implant was realized, and the gingival fullness and bone mass around the implant were significantly increased, achieving a relatively good therapeutic effect. To achieve long-term functional stability in implant repair, doctors and patients must collaborate. Doctors need to assess risks, design proper repair plans, and establish good follow-up. Early intervention is crucial. This paper presents a case of an end-stage dentition patient who, 14 years after full-mouth implant fixed prosthesis, developed peri-implantitis of varying degrees in individual sites. After combined treatment with non-surgical therapy, regenerative surgery, and resective surgery, satisfactory results were achieved.
