Abstract: Restoring grossly decayed molars following endodontic treatment presents both biomechanical and functional challenges, particularly when there is extensive loss of coronal tooth structure. This case report highlights the successful rehabilitation of severely damaged mandibular rst and second molars (teeth 46 and 47) in a 32-yearold female patient using custom-made cast post and core systems followed by full coverage porcelain-fused-to-metal crowns. Clinical and radiographic evaluation conrmed satisfactory endodontic treatment, but inadequate remaining coronal structure necessitated the use of custom posts for retention. Post space was carefully prepared, wax patterns were fabricated, and cast posts were luted using resin cement. Full coverage restorations were then placed to restore function and esthetics. Custom cast posts offer superior adaptation to irregular canal morphologies and better resistance to dislodgement compared to prefabricated posts, especially in posterior teeth subject to high occlusal forces. Careful post space preparation, preservation of apical seal, and incorporation of a ferrule design are critical for long-term success. This case emphasizes the importance of individualized treatment planning and appropriate post selection in restoring endodontically treated molars. The custom cast post and core system remains a reliable option for managing grossly decayed teeth with signicant coronal destruction, offering favorable prognosis and functional outcomes when executed with precision.
