Type: article
Abstract: Background: Oblique and spiral fractures of the metacarpals and phalanges are relatively common injuries that result from torsional forces and are prone to rotational malalignment. Optimal management remains debated, with various conservative and surgical techniques described. This study aimed to evaluate the clinical, functional, and radiological outcomes of treating oblique Methods: This prospective study included 24 patients with oblique or spiral fractures of the metacarpals and phalanges treated at Kafr El-Dawar General Hospital between May 2023 and May 2024. Postoperative protocols emphasized early mobilization. Patients were followed for a mean of 24 months. Outcomes were assessed using range of motion, Total Active Motion (TAM) score, Quick Disabilities of the Arm, Shoulder, and Hand (Quick DASH) score, and serial radiographs. Results: The mean age of patients was 29.4 years, with a male predominance (83.3%). Similarly, the mean Quick DASH score improved from 25.1 ± 11.7 at six weeks to 13.2 ± 6.5 at final follow-up (P < 0.001). The mean union time was 7.5 weeks (range 5–11). Better outcomes were observed in metacarpal compared to phalangeal fractures, proximal compared to middle phalanges, nonsmokers, and those with shorter surgical delay and immobilization periods. Conclusion: Mini lag screw fixation is a reliable and effective method for treating oblique and spiral metacarpal and phalangeal fractures, providing stable fixation, satisfactory functional recovery, and timely fracture union. Early surgery, limited immobilization, and careful management of risk factors such as smoking and infection are essential to optimize outcomes
