Prognostic impact of the number and Temporality of heart failure Hospitalisations: Analysis of a National healthcare database

Study design: Observational Study
Abstract: AimsTo describe relationships between the number as well as temporality of previous heart failure (HF) hospitalisations and outcomes of patients with reduced left ventricular ejection fraction (LVEF).Methods and resultsThis retrospective observational study analyzed data from the French national healthcare database for 730,052 adults who were identified as HF patients. Using a dedicated algorithm, 226,747 were selected as HF with reduced LVEF and 54504 had also at least one previous HF-related hospitalisation > 24 hours in the 2 years before the index date. This cohort was subdivided in several groups according to 1/the delay between the most recent HF-related hospitalisation to the index date, and 2/ the number of HF-related hospitalisations in the 2 years before. Mean age was 76 ± 15 years. Of all variables with a prognostic impact, the presence of a history of HF hospitalisation was most strongly associated with the risk of death. After adjustment on other predictive variables, patients with previous HF hospitalisation had higher mortality (HR 1.61 95% CI 1.56-1.65) and more all-cause hospitalisation (HR 1.34 95% CI 1.32-1.37) and HF hospitalisation (HR 2.51 95% CI 2.43-2.59). This higher risk was greatest in the subgroup of patients hospitalized for an HF event within the previous 6 months and gradually increased significantly with the number of previous HF hospitalisations.ConclusionThe increased risk of hospitalisation and death in patients with previous HF hospitalisation, particularly those recently and/or frequently hospitalized, merits particular attention to improve outcomes.
