Safety profile of MESMO® breast implants in breast reconstruction: A retrospective analysis

Abstract: ABSTRACT Background Breast implant-associated complications, including rare events like breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), raised concerns regarding implant safety. Micro-textured implants, such as the MESMO® implants, were developed to reduce these risks. However, limited data exist on the safety and performance of MESMO® in breast reconstruction, particularly in irradiated patients. Methods A retrospective analysis of 178 women who underwent direct-to-implant (DTI) breast reconstruction with MESMO® implants was conducted between 2018 and 2024. Data on demographics, treatments (including radiotherapy), and postoperative outcomes were collected. Statistical analyses were performed to assess the influence of various factors such as age and body mass index (BMI) and to identify the key endpoints, including rates of any complication, capsular contracture (CC, Baker grade III/IV), implant removal, and reoperation. Results The overall complication rate of MESMO® implants was 23.3% (CI = 17.8 - 30.4), with a probability of remaining free from major complications of 94.1%. The median follow-up was 26 months (range, 2 - 60 months). The overall complication rate is low, with the majority of events being minor and conservatively managed. MESMO® also exhibited a low rate of implant removal (2.7%, CI = 1.0 – 7.1). Radiotherapy did not significantly increase the risk of any complication. No cases of BIA-ALCL were observed. Conclusions MESMO® implants demonstrated a favorable safety profile comparable to that of other contemporary implants. Postmastectomy radiotherapy did not substantially impact outcomes, suggesting that MESMO® is a safe option for breast reconstruction. Further studies with larger cohorts are needed to confirm these findings.
