Abstract: A 38-year-old woman presented with fevers, lower abdominal pain, and an infertility history. She received IV antibiotics for presumed bilateral tubo-ovarian abscesses despite lacking risk factors.Inflammatory markers were elevated (CRP 126, WCC 16.5) with negative pelvic swabs and urine cultures.CT imaging demonstrated bilateral hydrosalpinx, inflammation suggestive of bilateral tubo-ovarian abscess (TOA) and a gas-containing collection which appeared in continuity with her sigmoid colon. She improved and was discharged with oral antibiotics.
