Approach to a symptomatic unruptured intracranial aneurysm presenting as status epilepticus: a case report and literature review
Year: 2025
Type: article
Study design: Case Report
Abstract: Unruptured intracranial aneurysms are mostly diagnosed after hemorrhage from another aneurysm, or incidentally when evaluating neurological symptoms such as headache, ischemic cerebrovascular disease, and cranial nerve palsy. Seizure as a presenting feature is unusual. We reported a case of a 39-year-old woman who suffered status epilepticus caused by an unruptured intracranial aneurysm. Head magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA), confirmed with cerebral angiography, revealed a large left internal carotid artery (ICA) saccular aneurysm. Mass effect resulting in direct compression causing seizures. Symptoms resolved after endovascular coiling was performed. We aim to present a case of seizure as a rarely considered manifestation of a symptomatic unruptured intracranial aneurysm. A treatment strategy prior to hemorrhage is essential.
