Abstract: We report a case of a 45-year-old man who developed a persistent inflammatory scalp reaction 1 year after undergoing synthetic hair implantation (Biofibre ® ). Histopathology revealed a foreign body granulomatous response with neutrophilic abscesses and multinucleated giant cells surrounding filamentous exogenous material. Clinical improvement was partial after systemic and topical anti-inflammatory treatment, though low-grade inflammation persisted. This case highlights the potential for delayed immune reactions despite the use of modern synthetic fiber technologies and reinforces the need for histological assessment in post-implantation scalp lesions.
