Abstract: Background: Renal tumor classification has evolved over time, incorporating advances in histological and molecular understanding. The World Health Organization (WHO) released an updated 2022 classification of genitourinary tumors, warranting a reassessment of previous diagnoses to improve characterization and prognostic accuracy in renal cell carcinoma (RCC).Objective: To retrospectively reclassify RCC cases using the WHO 2022 classification and International Society of Urological Pathology (ISUP) grading system, comparing them with original diagnoses and assessing their impact on staging and prognosis.Material and methods: A total of 93 RCC cases from the Department of Surgical Pathology at Centro Médico ABC (2013-2022) were analyzed. Two board-certified pathologists and a pathology resident reviewed the slides using a dual-head microscope, with immunohistochemistry performed in selected cases.Original diagnoses were compared with reclassification using WHO 2022 and ISUP criteria, and staging was updated according to the 8 th edition of the tumor-node-metastasis (TNM) system. Results: Clear cell RCC accounted for 70% of cases, chromophobe RCC for 11.8%, papillary RCC for 5.3%, and the remaining cases included rarer subtypes. There was an 86% concordance with the original diagnoses, with grade or subtype changes in 14% of cases.Reclassification using ISUP/WHO altered 16% of cases compared to the Fuhrman system, identifying sarcomatoid and rhabdoid differentiation in 7%.Staging was updated in 55 cases (59%), with 35% classified as pT1a.Conclusions: The WHO 2022 classification and ISUP system proved useful in refining RCC characterization and improving prognostic feature identification.Applying these criteria in retrospective studies allows for a more precise evaluation of tumor aggressiveness and contributes to the optimization of pathology databases.
