Guideline-directed Medical Therapy in Pregnancy and Lactation: Navigating Evidence Gaps and Clinical Application

Year: 2025
Type: article
Abstract: The management of a heart failure patient in pregnancy is complex and requires a multidisciplinary Cardio-Obstetric team to help coordinate and follow a patient from preconception to delivery. The medical approach to managing these patients is limited due to the teratogenicity of many commonly used heart failure medications and the limited data available for the pregnant population. In this paper, our goal is to create a useful guide for safe and practical medical management of the pregnant systolic heart failure patient by medication class. We will also highlight medical management throughout critical time periods of pregnancy from preconception through postpartum and lactation.
