Exosome-Based Therapies: Protocol Development for Androgenetic Alopecia
Year: 2025
Type: article
Abstract: Exosome-based therapies are emerging as promising, cell-free biologics in regenerative dermatology. For androgenetic alopecia (AGA), mesenchymal stem cell–derived exosomes modulate dermal papilla biology and the perifollicular niche through paracrine cargo (microRNAs, proteins) that engage Wnt/β-catenin, VEGF, angiogenic, and anti-inflammatory pathways. This Special Topic article outlines protocol development for AGA, emphasizing donor screening, manufacturing standardization, and clinical application parameters. The authors review biological rationale; summarize quality-control elements—rapid post-harvest processing, tangential-flow filtration, size profiling, exosomal markers (CD9, CD63, CD81), sterility and endotoxin testing; and propose treatment variables including microneedling depth, dosing, and retreatment intervals. The authors also identify mechanistic and clinical gaps and present a practice example: a 45-year-old male treated with 3 sessions of microneedling (1.5 mm) plus topical adipose-derived exosomes at 4-week intervals improved from 75 to 110 hairs/cm² without adverse events. Standardized protocols and transparent donor metadata are essential to reproducibility, potency definition, and regulatory readiness. The authors recommend a development path combining multicenter randomized trials with mechanistic substudies and harmonized pharmacovigilance to capture durability and safety. With rigorous sourcing, manufacturing, and application standards, exosome therapy may offer a biologically driven alternative or adjunct to finasteride, minoxidil, and surgery, potentially reshaping care pathways for AGA. Further standardization will enable broader clinical adoption.
