In Hydatidiform Mole: Prophylactic Chemotherapy Prevent Malignant Transformation

Year: 2025
Type: article
Abstract: Aims and objectives: To determine the therapeutic and preventive role of prophylactic chemotherapy in gestational trophoblastic diseases (GTDs) (H.Mole) in transformation to or development of malignancy. To determine its (P-Chem-Regimen) safety and efficacy.Nature: Well-designed, prospective, clinical, randomized controlled trial.Place: In the Departments of Obstetrics and Gynecology, IPGMER-SSKMH, Kolkata, West Bengal, India.Duration: More than 1 year.Materials and methods: In more than 35 patients, evacuation was carried out with the adjunct of P-Chem (Methotrexate + Vitamin-A + Actinomycin D) in Chemoprophylaxis Gr-A-Cases, and more than 35 patients (Gr-B-Controls) were treated conventionally without P-Chem.The sample after evacuation was examined for histopathology grading of the molar gestation. Results and analysis: Between the chemoprophylaxis group and control group, there were significant differences in mean age, the mean of gestational age (GA), the mean serum β-hCG, the mean time to diagnosed GTD in months, for treatment and the mean Karnofsky score. The parameters in forms of β-hCG, recurrence, the size of uterus, the theca-lutein cysts, the vaginal bleeding, the chest X-ray were normalized, but there were increased serum transaminase, low blood platelets, and neutrophil. There were meaningful association in the form of age, parity, the mode of delivery (MOD), P-Chemo administration, vomiting, alopecia, mucositis in the chemoprophylaxis group. Conclusion: From this study, it can be concluded that prophylactic chemotherapy has both potential and definitive roles in the management of molar pregnancy (hydatidiform mole) to prevent subsequent cancerous growth, later with minimal toxic effects and without death.
