Abstract: Dear Editor, We read with great interest the article "Comparison of Doppler Echocardiographic and Invasive Hemodynamic Methods for Optimizing Patients Receiving Cardiac Resynchronization Therapy" by Corut Guzel H. and colleagues (1). The comparison of two different methods for optimizing AV and VV delays in patients receiving cardiac resynchronization therapy (CRT) sheds light on a valuable topic that has received limited coverage in the existing literature. We congratulate the authors on this important contribution. The study is based on sound methodological and clinical foundations and successfully compares the acute and mid-term outcomes of invasive and echocardiographic methods. However, We believe there are some aspects that should be considered in interpreting the study's findings. First, while the 6-month follow-up period provides important data, it is known that the effects of CRT on long-term mortality and morbidity generally become more pronounced after 12 months (2). Therefore, longer-term follow-up data could more clearly reveal the differences between optimization methods and enable stronger conclusions to be drawn about the durability of the results obtained.
