Abstract: We report a rare case of extensive Stanford type B iatrogenic aortic dissection during complex percutaneous coronary intervention. This case highlights the importance of early recognition, cautious catheter and guidewire manipulation, and tailored imaging to guide management. • Extensive Stanford type B iatrogenic dissection during complex PCI • Dissection from right external iliac to left subclavian artery • IVUS and aortography used for detection and CTA confirmation • Avoid forceful wire/catheter advancement to reduce iatrogenic dissection risk
