Abstract: Background Acute Coronary Syndrome (ACS) is a common heart emergency, mainly caused by the rupture of coronary artery plaques and thrombosis. It has a significant impact on the psychological state of patients. Early diagnosis of ACS and the adoption of appropriate treatment plans are crucial for reducing the mortality rate of ACS. MicroRNAs (miRNAs) are widely present in cardiovascular diseases and are of great significance for their diagnosis. The main research focuses on the diagnostic and prognostic value of miR-1282 in patients with ACS, as well as the impact of psychological nursing on the recovery of ACS patients. Methods The study included 52 healthy controls and 145 ACS patients. MiR-1282 expression was measured by RT-qPCR. Its diagnostic and prognostic value were assessed using ROC and Kaplan–Meier analyses, while logistic and Cox regression identified independent risk factors for ACS onset and outcomes. Functional assays (CCK-8, Transwell, flow cytometry) evaluated cell viability, migration, and apoptosis. Results Serum miR-1282 was downregulated in ACS patients, particularly in AMI, compared to UAP, while its target QKI showed an opposite trend. MiR-1282 has high diagnostic value for ACS and is an independent influencing factor for the occurrence of ACS. MiR-1282 and three-level psychological nursing have independent prognostic value for MACE in ACS patients. In vitro experiments showed that overexpression of QKI reversed the promoting effect of miR-1282 on the growth vitality and migration of HCAEC. Conclusions MiR-1282 has a high clinical significance for the diagnosis of ACS and AMI. For patients with ACS, high levels of miR-1282 and receiving three-level psychological nursing are associated with a lower likelihood of developing MACE.
