Impact of Cardiovascular Complications on the Economic Burden in Patients with Hypertrophic Cardiomyopathy

Year: 2025

Study design: Observational Study
Abstract: Background About 2/3 of patients with hypertrophic cardiomyopathy (HCM) have obstructive hemodynamics (oHCM), and there is limited evidence about the complication burden, particularly for nonobstructive HCM (nHCM). Methods Claims data were analyzed (ICD-10 codes) to understand the impact of complications on the costs of care between 2016 and 2025. Complications, resource use, and costs were evaluated (mean, annual, per patient) for adult patients with HCM. Results We included 35,249 patients with oHCM (20,820 symptomatic) and 43,775 patients with nHCM (23,894 symptomatic). In symptomatic oHCM, complications were associated with increased mean annual inpatient admissions ( P = 0.001) and costs ( P = 0.022), outpatient visits ( P = 0.010), ER costs ( P = 0.006), and visits ( P = 0.005), urgent-care costs ( P = 0.022), and non-HCM-related prescription costs ( P < 0.001). In asymptomatic oHCM, complications were associated with increased mean annual outpatient visits ( P < 0.001) and urgent-care visits ( P = 0.005). In symptomatic nHCM, complications increased inpatient admissions ( P = 0.009) and costs ( P = 0.013), HCM-related prescriptions ( P < 0.001), and cost ( P < 0.001), non-HCM-related prescriptions ( P < 0.001) and cost ( P < 0.001), and other visits ( P = 0.005). In asymptomatic nHCM, complications increased inpatient admissions ( P = 0.041), emergency room visits ( P = 0.007), and costs ( P = 0.017), HCM-related prescriptions ( P = 0.002) and costs ( P = 0.049, and non-HCM-related prescriptions ( P < 0.001) and costs ( P = 0.006), and total costs ($60,619–$87,281; P = 0.022). Conclusion Patients with HCM incur considerable costs and health care resource use, which is further escalated upon the occurrence of cardiovascular complications, thereby escalating the economic burden of this disease despite available treatment regimens. Future research is warranted to determine whether early interventions reduce downstream complications and costs for patients with HCM.
