Editorial Comment: Robot-Assisted Radical Prostatectomy With the Toumai Surgical System: Step-by-Step Description of the First Clinical Case From a Multiplatform User
Year: 2025
Type: editorial
Abstract: New multiport robotic systems deserve careful evaluation beyond feasibility. Rocco et al1 report the first European robot-assisted radical prostatectomy using the Toumai platform, executed by a multiplatform surgeon with prospective step-wise assessment against European Association of Urology (EAU) Robotic Urology Section/OLV Robotic Surgery Institute metrics. The operation was completed without critical problems, with low blood loss and efficient docking, and the authors transparently provide console, operative, and docking times alongside trocar/docking schematics: useful details for early adopters. Two strengths stand out. First, feasibility is anchored to a validated intraoperative skills framework rather than narrative description, setting a higher bar for early device reports.2 Second, the manuscript explicitly addresses transition from existing systems to Toumai—information that matters for programs planning interoperability and training. Limitations are intrinsic to a single case: potential Hawthorne effects, no external rater, and uncertain generalizability across anatomies and teams. The logical next step is a multicenter prospective registry using device-agnostic European Association of Urology (EAU) Robotic Urology Section/OLV Robotic Surgery Institute metrics with predefined end points (safety/efficiency and early oncologic/functional outcomes), coupled with systematic capture of near-misses and usability and ergonomics signals. Reporting should follow guidance that makes causal intent explicit and language transparent in observational work, improving interpretability and preventing overreach.3 As competition returns to robotic urology, this report models the right essence: structured methods, reproducible metrics, and clear boundaries on inference. This approach is more informative than platform promotion.1-3
