Abstract: OBJECTIVE: Present a novel, minimally-invasive approach for closing persistent femoral artery pseudoaneurysms using a suture-mediated closure device. Demonstrate how the described technique avoids previously reported technical issues with suture-mediated closure of pseudoaneurysms. KEY STEPS: Use ultrasound guidance to visualize the pseudoaneurysm neck and guide access needle to puncture the neck at an oblique angle. Following neck puncture, advance the needle into the periarterial tissue and then the femoral artery. Advance guidewire, dilate and pass a suture-mediated closure device over guidewire, deploy a foot-plate, and tie the suture following manufacturer instructions. POTENTIAL PITFALLS: Good ultrasound technique is required in order to ensure passage of the access needle through the pseudoaneurysm neck. TAKE-HOME MESSAGES: Persistent femoral pseudoaneurysms can be effectively closed with percutaneous suture-mediated closure devices without the need to cross the pseudoaneurysm neck as previously reported. This technique can reduce procedure time, radiation, and contrast usage and be particularly useful in closing pseudoaneurysms with long or tortuous necks.
