1133 Efficacy and Safety of Hyaluronic Acid Fillers for Horizontal Neck Lines: A Systematic Review and Meta-Analysis
Year: 2025
Type: review
Study designs: Systematic Review, Meta-Analysis
Abstract: Background Horizontal neck lines are a common aesthetic concern among ageing populations. Hyaluronic acid (HA) fillers are increasingly used in this region, yet their effectiveness, safety, and optimal application strategies remain under-investigated. Method This systematic review and meta-analysis were conducted in accordance with PRISMA guidelines and registered with PROSPERO. A comprehensive search of Ovid MEDLINE and Embase was performed up to May 2024. Studies investigating HA fillers for horizontal neck lines in adults were included. Data were extracted on patient-reported outcomes, instrumental assessments, injection techniques, and complications. Risk of bias was evaluated using the Joanna Briggs Institute (JBI) tools. Results Eleven studies (n = 429) were included, comprising ten cohort studies and one randomised controlled trial. Pooled analysis demonstrated significant improvement in the Global Aesthetic Improvement Scale (mean score: 1.75, 95% CI: 1.53–1.96) and wrinkle grading (mean difference: 1.43, 95% CI: 0.72–2.15). Instrumental assessments confirmed improvements in hydration, elasticity, and skin tone. Histological and ultrasound findings supported dermal thickening and matrix remodeling. The pooled retreatment rate within 12 months was 50%. The complication rate was 14%, with only mild adverse effects reported. Patient satisfaction was consistently high across studies. Conclusions Hyaluronic acid fillers represent an effective and well-tolerated option for treating horizontal neck lines, showing consistent improvements in both aesthetic appearance and skin quality. While current evidence is encouraging, further research is warranted to refine injection techniques, evaluate long-term durability, and patient preferences regarding downtime and retreatment.
