Minimally invasive replacement of a scleral-fixated intraocular lens suture using a sequential pull-through suture exchange

Abstract: Suture loosening and erosion are well-documented and relatively common postsurgical complications of scleral fixated intraocular lens (IOL) implantation. These complications may result in dislocation of the IOL, leading to visual symptoms. In these cases, ophthalmologists perform suture retrieval and replacement through a transscleral, ab externo approach. However, as with any intraocular surgery, there are many risks associated with this procedure including endophthalmitis, inflammation, and retinal detachment. We present a novel technique for scleral suture exchange that bypasses intraocular entry while maintaining surgical efficacy. In short, a sequential pull-through of sutures is performed, allowing the original suture to ultimately be replaced through externalized maneuvers. Overall, this novel technique may provide a safer and more efficient alternative for managing suture-related complications in patients with scleral-fixated IOLs.
