Abstract: Background: Transvaginal ultrasound is safe in situations of placenta previa and en-ables a thorough evaluation of the lower uter-ine region. Transabdominal ultrasound and transvaginal ultrasound are complementing for the detection of unusually invasive pla-centas.Objectives: To assess the predictive role of ultrasound in detection of placental invasion in placenta previa. Methodology : Eligible patients underwent a methodical scan of the entire placenta us-ing color flow visualization and gray-scale two-dimensional (2D) ultrasound, and all of the pregnancies included in the study were terminated by a cesarean delivery at our medical facility. Sensitivity and specificity analyses were done to determine the detect-ability of the investigation. Results: Fifty-seven pregnant women who had persistent placenta previa (after 28 weeks of gestation). 2D ultrasonography has sensitivity and specificity to detect cases of placental invasion up to 88.57% and 90.9%, respectively. Colored Doppler diagnosed all cases (100%) of the morbidly adherent pla-centa (n=35/35). Dilated vessels over the pe-ripheral sub-placental region and hyper-vas-cularity of the uterine–bladder interface were the most accurate indicators for detection. Conclusion: Color Doppler ultrasound, when aberrant vasculature is present, offers the best combination of sensitivity and spec-ificity. Myometrial thinning was the most sensitive sign, while intra-placental lacunae were the most specific indicator.
