Abstract: Background. The impact of cytoreductive surgery on survival of patients with FIGO stage IVB endometrial cancer is unclear. Patients and methods. We performed a multicenter, retrospective population-based registry study of 6582 patients with endometrial cancer. The primary outcome measure was disease-free survival.Results. After exclusions, the final study involved 294 patients with FIGO stage IVB endometrial cancer. Seventy eight (26.5%) were treated with surgery only, 53 (18.0%) conservative, and 163 (55.4%) received combination therapy. The median follow-up was 31 months (range 0-224 months). The combination therapy was associated with an improved median progression-free (p=0.003) and overall survival (p<0.001). The combination treatment was a favourable prognostic factor even after adjustment for age at diagnosis, histological grade and type, and number and site of distant metastases. The hazard ratio for disease events among patients receiving surgery plus non-surgical therapy compared with non-surgical therapy alone was 0.58 (95% CI: 0.39–0.86; p = 0.007). The surgical cytoreduction in combination with conservative treatment was also associated with improved overall survival compared with conservative treatment alone 0.49 (95% CI: 0.33-0.73, p < 0.001).Conclusions. Cytoreductive surgery is associated with improved patient survival in FIGO stage IVB endometrial cancer.
