Rare vascular brain lesions: a case report of cerebral proliferative angiopathy management

Year: 2025
Type: article
Study design: Case Report
Abstract: Introduction and Importance: Cerebral proliferative angiopathy (CPA) is a rare vascular disorder characterized by the arteriovenous malformation (AVM) within the brain. These abnormal vessels can lead to a variety of neurological symptoms, including seizures, stroke, and motor deficits. Due to the risk of permanent neurological damage associated with invasive treatments, conservative management strategies are often employed. Case Presentation: In this case, a 75-year-old woman presented with right-sided focal seizures, altered mental status, and hemiplegia. Imaging studies revealed CPA, showing diffuse vascular proliferation without the presence of a dominant feeding artery. The patient received conservative treatment, including antiplatelet therapy and lifestyle modifications, to manage her symptoms and prevent further complications. Clinical Discussion: This case emphasizes the importance of conservative management in geriatric patients with CPA, as using invasive approaches carries a high risk. The accurate diagnosis of such cases is also important for not to provide inappropriate interventions. Conclusion: The conservative management is an option for the CPA patients who cannot undergo surgery. There is a requirement for further studies to establish the guidelines for the management of this disorder.
