Abstract: Pleomorphic adenoma of the submandibular gland is rare, particularly in adolescents, accounting for a small fraction of head and neck tumors. This case report presents a 16-year-old female with a painless, gradually enlarging right submandibular mass noted over four years. Diagnostic evaluation using fine needle aspiration biopsy and contrast-enhanced CT confirmed pleomorphic adenoma. The patient underwent partial submandibular sialoadenectomy with complete tumor excision (5 × 3 × 2 cm), preserving surrounding vital structures. Histopathological examination verified the diagnosis. Recovery was smooth, without nerve damage, xerostomia, or cosmetic issues. The patient was discharged on day four and showed satisfactory healing at one-month follow-up with no recurrence. This case underscores the importance of considering pleomorphic adenoma in adolescents with persistent neck masses. FNAB and CT are valuable for diagnosis, and early surgical intervention ensures excellent outcomes. Regular follow-up is essential to monitor for recurrence and maintain function.
